General Graduate Forms

Application for Degree (Diploma)

Log into your  MyNMSU > "Student" tab on the top > "Your Student Profile Page" under Student > "Apply to Graduate" on the left.
Follow the on-screen prompts of instructions to submit your application.

FOR: All current graduate students (Required)
PURPOSE: To have your your degree (inlcuding Graduate Certificate) awarded. For more information please contact  diplomas@nmsu.edu or call 575-646-4796.

Change of Admission Semester, Change of Campus, Cross Enrollment Petition, Graduate Assistant Part-Time Enrollment Waiver, Leave of Absence, Program of Study Change, and Transfer of Credit :

Step-by-Step Guide:

  1. Log in to your application portal page .
  2. Select the desired/current application and access the forms under the "Current Graduate Student Forms" section.
  3. Click the Form link, complete, and submit it.

Note: Make sure you choose the correct application Form in Step 2. The Form you submit will be linked to that specific application.

Check Your Form Status: After submission, view the status under the "Previously Submitted Form Status" section for each Form.

Degree Audit Exception Form

FOR: All current graduate students whose program is on the  STAR-Audit system
PURPOSE: If there is a change in STAR-Audit, this Form needs to be submitted

Request for Letter of Completion

Request for Letter of Completion Form

FOR: Doctoral, Masters, and Educational Specialist Students
PURPOSE: If a student’s employer requests proof of completion of course work, a student may use this Form

FOR: Ph.D. Students in the College of Engineering
PURPOSE: The Form documents the requirements to publish and must be completed and attached to the Doctorate of Philosophy Examination Form by only College of Engineering Ph.D. students

Doctoral Dissertation Title Submission Form

Doctoral Dissertation Title Submission Form

FOR: All Doctoral students
PURPOSE: Doctoral students must complete this Form when completing paperwork for degree application. The information supplied on this Form is used by the Registrar’s Office to include the doctoral student in the commencement ceremony program. The student must be listed in the program to be part of the ceremony.

Report of Results for Doctoral Qualifying Examination [FACULTY USE ONLY]

FOR: For Ph.D. students who enter the Graduate School with little or no previous graduate experience, but wish to proceed directly to the doctorate
PURPOSE: This Form needs to be completed by the Program Director or Department Head to indicate the results of the Doctoral Qualifying Examination

Student Records Forms

The Student Records Office Forms Inventory features numerous student forms pertaining to course registration, schedule changes, semester withdrawals, tuition waivers, and more. Please note that many of these forms feature signature lines for the student's Academic Dean and Associate Academic Dean, which refers to the Deans and Associate Deans in the academic colleges and not the Graduate School Dean. Please work with your academic program, department, and college on these Student Records Office Forms.
